Our Mission

Slym Wellness Clinic was established out of passion to pursue personal health and wellness. Our goal at Slym is to be a liaison on your journey to a healthier lifestyle. Our providers are certified in Family Medicine and Medical Weight loss. We use the most current, evidence based information available to make sound clinical decisions while utilizing the latest medications to assist in your lifestyle change. Studies demonstrate that a weight loss of 5-15% of total body weight for a person who is overweight or obese can reduce deadly risk factors for many diseases, including diabetes and cardiovascular diseases.

Our Vision

Slym Wellness Clinic providers are proud to be part of the National Registry of Certified Medical Examiners and we serve our community by offering Department of Transportation physical examinations for the CDL drivers, whether it’s your initial, annual or biannual DOT physicals, we are here to provide quick and easy service.

Slym wellness has not forgotten about our children athletes and those entering school. Sports and school physicals offered both during season and off season and comply with current school and sports regulations

Our Office

Our office is conveniently located between San Jose Blvd and Interstate 95 on Baymeadows Rd. We are equipped with the latest equipment for DOT, sports and school physicals. 

We are proud to offer telemedicine visits for our Wellness Services at you convenience.

The whole process was fast, easy, and smooth. Appointment fees are the lowest i’ve seen for trying to get services like this not covered by insurance. They try to make sure you completely understand the process and the available treatments. Highly recommend as it is a cheap way to inform yourself of the options out there

Very happy to have found a knowledgeable practitioner who takes the time to listen and answer all of my questions (and there were a lot!). He went over my current labwork and past medical history. Jason also explained the science behind the medicine and how to administer it correctly. Hands down Jason is great, I completely recommend him.
