Why the ADAM Questionnaire Matters at Slym Wellness Clinic

At Slym Wellness, we turn a simple symptom screen into a personalized plan. Your responses help our providers decide whether to:

  • Order targeted labs (total & free testosterone, SHBG, LH/FSH, thyroid markers, metabolic panel)

  • Review medications, sleep, nutrition, and training that influence hormones

  • Coordinate with our clinical nutrition team to protect lean mass, energy, and metabolic health

  • Discuss evidence-based options for restoring balance and quality of life

Our goal isn’t just to spot low T—it’s to understand why you feel the way you do and map a path forward that fits your life.

Understanding the ADAM Questionnaire for Men's Hormone Screening

IMPORTANT DISCLAIMER: This questionnaire is for educational screening only and does not diagnose medical conditions. A licensed Slym Wellness provider must review your health history and any lab results before recommending treatment.

The ADAM Questionnaire (Androgen Deficiency in Aging Males) is a quick self-assessment used to screen for symptoms that may be associated with low testosterone (androgen deficiency). It helps determine whether a professional hormone evaluation may be appropriate.

Answer ten Yes/No items related to libido, energy, strength/endurance, mood, sleepiness, erections, exercise capacity, work performance, and enjoyment of life.
A positive screen typically occurs if you answer “Yes” to Question 1 (libido) or Question 7 (erections), or if you have three or more “Yes” answers in total. Use your result as a prompt to speak with a provider—it is not a diagnosis

Positive Screen
Your answers suggest possible androgen deficiency. A Slym Wellness provider may recommend lab testing and a clinical evaluation. Lifestyle, sleep, and nutrition support are addressed alongside any medical therapy.

Borderline Screen
Mixed signals (e.g., exactly two “Yes” answers, with Q1 & Q7 “No”). We’ll review sleep quality, stress, training load, and nutrition—and decide whether labs are appropriate now or after targeted habit changes.

Negative Screen
Your answers don’t strongly suggest androgen deficiency. If symptoms persist, we can evaluate thyroid, adrenal, sleep, and nutrition factors and set a follow-up plan.
