OUR LEGAL RESPONSIBILITIES

We are required by law to provide you with this notice, which outlines how we may use and disclose your protected health information (PHI), your rights regarding your PHI, and our legal duties with respect to your PHI. In accordance with HIPAA regulations, we are legally obligated to maintain the privacy of your PHI and provide you with notice of our legal duties and privacy practices.

We are committed to complying with all applicable federal, state, and local laws, including the Health Insurance Portability and Accountability Act (HIPAA) and LegitScript Certification Standard #9. We are required by law to:

Maintain the privacy and security of your PHI.
Provide you with a notice of our legal duties and privacy practices regarding your PHI.
Abide by the terms of this notice currently in effect.
In addition, we are required to notify you promptly if a breach occurs that may have compromised the privacy or security of your PHI.

We reserve the right to change our privacy practices and the terms of this notice at any time, provided such changes are permitted by law. If we make significant changes to our privacy practices, we will promptly revise this notice and notify you of these changes. The new notice will be available upon request, in our office, and on our website.

HOW WE MAY USE OR DISCLOSE YOUR PROTECTED HEALTH INFORMATION

The following examples describe how we may use and disclose your protected health information in accordance with HIPAA regulations. Please note that not every use or disclosure in a particular category will be listed, but all permissible uses and disclosures fall within these categories.

Treatment: We may use and disclose your PHI to provide, coordinate, or manage your healthcare and any related services. This includes sharing your PHI with other healthcare providers, trainees, and staff involved in your care.

Payment: We may use and disclose your PHI to obtain payment for the healthcare services we provide to you. This may include providing information to your health insurance company or a third party responsible for paying for your care.

Healthcare Operations: We may use and disclose your PHI to support the business activities of our practice. These activities include, but are not limited to, quality assessment, employee review, training of students, licensing, and conducting or arranging for other business activities.

Business Associates: We may share your PHI with third-party business associates who perform certain services for our practice, such as billing or legal services. We have written contracts with these business associates that require them to protect the privacy of your PHI.

Marketing: We may use your PHI to provide you with information about treatment alternatives, other health-related benefits and services, or to contact you regarding appointments. You have the right to opt out of receiving such communications.

Other Uses and Disclosures: Any other use or disclosure of your PHI will be made only with your written authorization. You may revoke this authorization in writing at any time, except to the extent that we have already taken action based on your authorization.

YOUR RIGHTS REGARDING YOUR PROTECTED HEALTH INFORMATION

You have several rights concerning your PHI, including the right to access, amend, and restrict certain uses and disclosures of your PHI. You also have the right to receive an accounting of disclosures and request confidential communication methods. All such requests must be made in writing to the contact person listed at the end of this policy.

Complaints: If you believe your privacy rights have been violated, you have the right to file a complaint with our office or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
