Jason Jacobson is a tenured, board certified Family and Emergency Medicine Advanced Practice Provider who earned his Baccalaureate Degree in Nursing from Chamberlain College of Nursing in Phoenix, AZ. After graduating Magna Cum Laude, Jason went on to serve in the Emergency Departments of the Phoenix and Jacksonville communities from trauma to urgent care. Graduating at the top of his class with his Masters Degree from Jacksonville University, in Jacksonville Florida, Jason has continued to serve communities in Philadelphia, Baltimore, Lake City, FL and Jacksonville providing primary, secondary and tertiary care. While visiting people in their homes, in primary clinics or at their bedside, Jason has worked with thousands of patients throughout his career and has consistently demonstrated one universal idea that resonates in his practice; provide patients with the best possible care while respecting the boundaries, limitations and needs of the individual.
